Independent Living Specialist/Deaf Services

Job Description:

To establish and work with consumers on personal goals for independence, including advocating for, and the coordination of, appropriate services for increased independence in the home and community.

 Duties:

1.   Evaluate the independent living needs of persons with disability who contact OSCIL for assistance and set priorities for service provision.

2.   Provide direct IL services to consumers, in consumer’s homes and at the OSCIL office (Information and Referral, Direct Skills Training, Advocacy and Peer Support) .

3.   Assist consumers with connection to community resources, transportation and housing options, and assist with the acquisition of assistive technology, as indicated in the consumer’s IL Plan.

4.   Provide driver’s education training one-to-one or in small groups to prepare consumers for the written test to obtain driver’s permit.

5.   May be required to work as a team with other OSCIL staff in the provision of services, assisting with communication clarification.

6.   Advocate for and coordinate appropriate services for increased independence in home and community.

7.   Regularly document all activity into agency’s data management System and provide monthly or quarterly reports to the Program Director, as requested.

8.   May be required to provide crisis intervention.

9. Other duties as indicated by the Program Director.

Qualifications:

1.     Must be fluent in American Sign Language (ASL) and have an understanding of Deaf culture.

2.     Minimum of Associate’s Degree in Human Services field.  Will consider candidate without degree with relevant work experience.

3.     Must have prior experience working with the Deaf community.

4.     Two or more years’ experience in addressing the needs of people with disabilities.

5.     Must have a valid driver’s license and be willing to travel to consumer homes to provide service. (OSCIL serves state-wide)

6.     Must possess excellent written communication skills, and be comfortable speaking in front of small groups.

7.     Must be able to work independently and under pressure and possess skill at setting priority for task accomplishment.

8.     Possess an understanding of the laws that effect persons with disability helpful, but not required.

9.     Must be able to pass a BCI check.

Hours:  Part-time position:  25 hours per week

Performance Evaluation and Probationary Period

A Performance Evaluation will be conducted at the end of the first six months of employment (Probationary Period) and annually thereafter.

   Occupational Therapist Assistant (Per Diem Position)

 

Job Description

Assist persons with severe disability in obtaining assistive technology/adaptive equipment and home modifications to address access issues or aid in performing activities of daily living. 

Duties:

Conduct in-home intakes and assessments of consumers requesting Assistive Technology, Home Modifications and/or general home safety assessments; assist consumers in identifying alternative funding sources for the purchase of devices and/or home modifications; complete documentation as may be required for funding source. 

  •  Complete necessary paperwork required for the attainment of AT/Home Modification.

  •  Keep accurate, up-to-date consumer records, including all documentation required by OSCIL and referral source.

  •  Other duties as assigned by the Program Director or Executive Director.

Qualifications:

§         Occupational Therapist Assistant training preferred, but not required.

§         Certified Aging in Place Specialist training helpful, but not required.

§         Possess driver’s license and reliable vehicle for home visits.

§         Knowledge of available assistive technology/devices available to persons  with disability.

§         Ability to work independently and possess skill at setting priorities for task accomplishment.

§         Knowledge of disability related issues either through previous work or personal experience.

§         Some knowledge of home modifications and understanding of architectural drawings is helpful.

§         Possess excellent written and verbal communication skills as demonstrated by previous work experience to   accurately convey needs of consumer and to articulate issues.

§         Must be computer literate; knowledge of Microsoft Office software helpful.

Salary:  Per Diem reimbursement will be determined by experience, credentials, and financial consideration of OSCIL.

Hours: To be negotiated (approximately 9 – 15 hours per week)
