We want to hear from people who have received our services!

Please take a few minutes to complete our quarterly Consumer Satisfaction Survey. Your responses help us to understand how we are doing and to continue to provide high-quality services to people with disabilities in Rhode Island.

Deadline to respond is February 19, 2022

Only those who have received services from OSCIL between October 1 – December 31, 2021 should fill out this survey.

If you did not receive services during this time period, please disregard.
